Program Application – This is a link to the online application and PDF version.
Resident Lease Agreement – This is the lease that each member signs to become a member and live in the shared home. This lease is the legal basis governing the resident’s tenancy. A Background Check by KHOC with a $20 fee is required before signing the lease.
Recovery Home Resident Handbook – This handbook outlines the rules and policies for living in KHOC’s transitional Recovery Homes. Recovery Home residents sign a Recovery Home Program Agreement.
